We currently accept assignments for Medicare, and various PPO's and HMO's, but you will be
required to pay any co-insurance and unmet deductible.  We ask that you come prepared to pay
for charges at the time services are rendered.  We will file insurance documents for the patient.
In all cases, we will send you a refund check if payment is sent to us and it is in
excess of the balance due.  Please let us remind you that our services, and those of our
staff, are provided directly to you and not to your insurance company.  You are ultimately
responsible for charges incurred through our office should your insurance company fail to
pay or delay payment.
